The set + name check is some kind of "fuzzy". For a possible rename it uses the most likely setname which doesn't have to be 100% correct. In your case this new name already exists. To prevent overwriting, ClrMamePro won't rename the files. Use Scanner->Advanced->Move not renamed sets to backup instead and let fixmissing or the rebuilder pick up the files from there...
