A set is a collection of roms. Like a folder on your harddisk. It can contain files and subfolders. So a set can have roms, samples, diskimages...and now a set can also have subfolders.
Like:

pacman\rom1
pacman\rom2
pacman\subfolder1\rom1
pacman\subfolder2\rom1
pacman\subfolder1\SubSubfolder1\rom1

etc...It's not needed for MAME, since MAME sets don't have subfolders.
Other systems have sets with folder structures. Image a datfile for a floppydisk of an Amiga system for example. You might find subfolders on that disk containing fonts or a subfolder for tools etc. If you want to keep one disk as a set, you have to keep that folderstructure. With ClrMamePro before 3.50 it wasn't possible to check/rebuild such sets, since ClrMamePro didn't look in set-subfolders. Everything was plain root based. Now it's possible and a lot datfile creators/rom collectors jump happily through their room.

No need for a handedit datfile. Dir2Dat supports subfolders...and it's the job of datfile writers to use or not to use them. They only use them when they are necessary of course.
