The naming and folder (=setname) of a rom is given by the datfile. The standard storing scheme is rompath\setname\file 1... file n (or rompath\setname.zip /.7z/.rar. So the rebuilder always creates setname.zip or the setname folder. If you want to specify another pre-folder you can use rebuilder advanced options to do so.

Forget about the miss/have lists in the scanner...they are quick but only check for the availability (and not correctness) of sets. Enable the chd scan option and you see a much more detailed list of chd specific errors.
This in combination sounds a bit like you want to automatically move chds to their correct position. You can do this by simply putting the chd into a rompath, the chd/name/unneeded fixes will automatically create a setfolder for it and moves the chd to its correct place.
