Most likely you have fixmissing enabled. So it'll add missing files from available addpaths (and from existing sets if you have an advanced scanner option enabled) to:

a) correct sysdefpaths (if fixmissing usage for sysdefpaths is enabled)
b) to the belonging existing set
c) if the set doesn't exist yet, it'll be created in the first rompath in the rompath order
