Can't you set parental options on consoles these days? Games should be able to detect these settings automatically and remove blood or whatever for kids and keep it in for adults.

I could be wrong, but I think if the content is on the disk it affects the rating.
I remember Rockstar had to sticker all the games on the shelf with Hot Coffee to AO. (not that I ever saw one, but remember reading about it) Then they sent out new disks with the content removed.
