If your pivot monitor is LCD, there is no hope to achieve what you want. Closest you can get is using the D3D renderer and running all games in the LCD's native resolution. Play around with the D3D stretching and filtering options to get the sharpest possible image.
If you monitor is a CRT, use Direct Draw and disable Hardware Stretch (You can also use the D3D renderer to achieve the same thing but there are a lot more settings to configure and I can't remember what they are right now ;-). That will get rid of the filtering, but most games won't fill up the screen. To remedy this, you add custom resolutions that matches the games native resolution (or double their resolution, eg. 256x224 = 512x448, if your monitor can't handle the lower resolutions or you want to use Scanlines). MAME will pick up on them and use them if you leave the resolution setting on 'auto'.
If you have an Nvidia card, you can add custom resolutions directly from the driver control panel. They will be available instantly, you don't need to reboot. Remember to restart Mame32FX so it can enumerate your new resolutions.
