To start a game you just double click it or click it and select play.
MameCE is preprogrammed with the directories that it will see ROM files in. It is set to the following:
\Program Files\MameCE\Roms\
\Storage Card\Program Files\MameCE\Roms\
\My Documents\
\My Documents\Roms\
